<p><b>Hazard: </b>The glass lid used with the 5-quart covered saut&#233; pan can crack, break or shatter, posing a laceration hazard to consumers.</p>
<p><b>Incidents/Injuries: </b>Meyer has received 65 reports of broken or shattered lids. No injuries have been reported.</p>
<p><b>Description: </b>The defective lid is 11 inches in diameter with a rubber and stainless steel handle and a metal rim. The code &quot;IMCP1108&quot; is stamped on the outside of the metal rim. The lid is part of the 13-Piece Circulon Premier Professional cookware set. The cookware is aluminum and stainless steel with rubber and stainless steel handles. In addition to the covered saut&#233; pan, the sets include: 1-quart and 3-quart saucepans, a 4-quart saucepot, an 8-quart stockpot, 8.5-, 10- and 12-inch French skillets and four additional glass lids.</p>
<p><b>Sold Exclusively at: </b>Costco retail stores and Costco.com from about October 2011 through March 2012 for about $$200 for the complete set.</p>

<p><b>Hazard: </b>Metal fatigue can cause the handlebar to break away during use, posing a risk of laceration from exposed metal surfaces or other injury from a fall.</p>
<p><b>Incidents/Injuries: </b>Aqua-Leisure has received four reports of handlebars breaking from the metal connection joint during use. No injuries have been reported.</p>
<p><b>Description: </b>This recall involves First Fitness Kid&apos;s First trampolines with handlebars. The child-size toy trampolines have a red and blue metal handlebar, a blue nylon deck guard and a black jumping deck. &quot;First Fitness&quot; is embossed on the jumping deck in white letters. The trampolines can be identified by model number FF-6902TR and Toys R Us SKN 491463. The model and store numbers can be found on the lower right corner of the back of the packaging. A sewn-in tag on the bottom of the deck lists the factory date code of five numbers followed by &quot;GLTX.&quot;</p>

<p><b>Safety 1st Toilet and Cabinet Locks Recalled Due to Lock Failure; Children Can Gain Unintended Access to Water and Dangerous Items</b></p>

<p><b>Name of Product: </b>Toilet and Cabinet Locks</p>
<p><b>Units: </b>About 183,000 toilet locks and 685,000 cabinet locks</p>
<p><b>Importer: </b>Dorel Juvenile Group (DJG) Inc., of Columbus, Ind.</p>
<p><b>Hazard: </b>Young children can unexpectedly disengage the toilet locks and gain access to water in the toilet, posing the risk of drowning. The cabinet locks are being recalled because young children can disengage the lock, posing the risk of injury from dangerous or unsafe items stored in the cabinet.</p>
<p><b>Incidents/Injuries: </b>DJG has received 110 reports of toilet locks that did not adequately secure the lid, including eight reports of children, under the age of two, who were able to disengage or break the lock. In addition, DJG has received 278 reports of cabinet locks that did not adequately secure the cabinet, including 71 reports of children between the ages of eight months and five years old who were able to disengage the cabinet locks. In one of the reported incidents, a 13-month-old boy swallowed small, toxic beads from a craft kit. The child was admitted to the hospital, observed overnight and released the next day.</p>
<p><b>Description: </b>This recall involves Safety 1st Sure Fit toilet locks with model numbers 48003 and 48103. The toilet lock is attached to the tank behind the lid and is intended to prevent a child&apos;s access to the toilet bowl. This recall also involves Safety 1st cabinet slide locks with model numbers 12013 and 12014. The cabinet slide lock is attached to cabinet knobs or handles to prevent access to the contents of the cabinet. Model numbers can be found on the back of the locks.</p>
<p><b>Sold at: </b>Bed, Bath and Beyond, Burlington Coat Factory, Great Beginnings, Home Depot, Target and Walmart from January 2005 through April 2010 for between $$8 and $$20 for the toilet locks, and from January 2000 through March 2009 for between $$2 and $$11 for the cabinet locks. Amazon.com sold both locks through April 2012.</p>
<p><b>Manufactured in: </b>China</p>
<p><b>Remedy: </b>Consumers should immediately remove the recalled locks and contact DJG for a free replacement lock of a different model. When removing the recalled locks, consumers are urged to immediately store dangerous items out of reach of children and to prevent unsupervised access to bathrooms.</p>

<p><b>Five Retailers Agree to Stop Sale and Recall Tots in Mind Crib Tents Due to Strangulation and Entrapment Hazard; One Death and Serious Brain Injury Reported</b></p>

<p>The U.S. Consumer Product Safety Commission (CPSC) and five retailers are announcing a voluntary recall to provide refunds to consumers who own crib tents and play yard tents made by Tots in Mind, Inc.</p>
<p>CPSC is warning parents and caregivers who own these products that infants and toddlers are at risk of serious injury or death due to strangulation and entrapment hazards presented by these products. Tots in Mind recalled the play yard tents in July 2010 and offered a repair kit that is no longer available. The company is no longer in business and has stopped all sales. CPSC staff urges parents and caregivers to stop using these crib tents and play yard tents immediately. <b>Do not attempt to repair these products.</b></p>
<p>CPSC is aware of 27 tent failures including one fatality and one serious injury that occurred between January 1997 and April 2012 from crib tents and play yard tents made by Tots in Mind, Inc. In 2008, a two year old boy died after becoming entrapped between the bottom rail of a play yard tent and the top rail of a play yard. The fatality was reported in a prior recall with the firm in July 15, 2010.</p>
<p>In 2007, a two year old boy sustained a catastrophic brain injury when the crib tent affixed to his crib tent inverted and the product&apos;s broken rod trapped him at the neck. The remaining 25 reports to CPSC involved inverted crib tents-entrapments between the tent and the crib/play yard or failures of the tent fabric and zippers. Three of these 25 reports also resulted in injuries; in one such case, a parent reported finding her child turning blue and entrapped between the product and the top rail of the play yard.</p>
<p>Since Tots in Mind, Inc. is out of business, retailers who sold these products have stepped up to offer refunds or store credit to consumers. The crib tents and play yard tents can present an entrapment and strangulation hazard to infants and toddlers if the dome portion inverts inside the crib or play yard, or if the product becomes partially detached from the crib or play yard. The recalled products were sold at numerous retail stores including Bed Bath &amp; Beyond/Buy Buy Baby, Burlington Coat Factory, Toys R Us/Babies R Us, Walmart and online on websites including Amazon.com, for between $$60 and $$85.</p>
<p>Consumers should contact the store listed below where the crib tent was purchased to receive either a refund or store credit, depending on the retailer. If consumer is unsure of where the crib tent was purchased, see return policy for individual retailers below on their website:</p>

<p><b>Hazard: </b>The jackets have a drawstring through the hood which poses a strangulation hazard to children. In February 1996, CPSC issued guidelines about drawstrings in children&apos;s upper outerwear. In 1997, those guidelines were incorporated into a voluntary standard. Then, in July 2011, based on the guidelines and voluntary standard, CPSC issued a federal regulation. CPSC&apos;s actions demonstrate a commitment to help prevent children from strangling or getting entangled on neck and waist drawstrings in upper outerwear, such as jackets and sweatshirts.</p>
<p><b>Incidents/Injuries: </b>None reported</p>
<p><b>Description: </b>This recall involves boys&apos; long-sleeved zip-front hooded jackets. The jackets were sold in three colors: army green, aqua green and black and have a fake fur lining. The jackets have one of four designs: the number &quot;2&quot; or the letter &quot;B&quot; embroidered on the left front chest; or the word &quot;ROCK&quot; or &quot;AB Sportwear&quot; embroidered across the chest. The jackets were sold in sizes 4-7 or S, M, and L. Either &quot;SX&quot; or &quot;QH&quot; appears on the label stitched at the neck. The jackets have model numbers: 1006, 1029, 1058 and 1061, however the model number only appears on the original hang tag and not on the product.</p>
<p><b>Sold exclusively at: </b>dd&apos;s Discounts stores nationwide from August 2011 through December 2011 for about $$8.</p>

<p><b>New CPSC Data Show Child Drownings In Pools and Spas Still A Leading Cause of Death; Pool Safely&apos;s First Splash Event to Focus on Vulnerable Populations</b></p>

<p>FORT LAUDERDALE, FL - Today, the U.S. Consumer Product Safety Commission (CPSC) kicked off the summer swimming season and the third year of the Pool Safely: Simple Steps Save Lives campaign.</p>
<p>This year, Pool Safely&apos;s focus is on populations most at risk of drowning, including children younger than 5 years old who represent nearly 75 percent of child drowning fatalities and African American and Hispanic children between the ages of 5 and 14 who drown at higher rates than white children, according to the Centers for Disease Control and Prevention.</p>
<p>Data from USA Swimming indicates that 70 percent of African American children and 62 percent of Hispanic children cannot swim, making them especially vulnerable populations.</p>
<p>&quot;CPSC&apos;s Pool Safely campaign has worked to prevent countless drownings, and we will continue to work to save even more lives this year,&quot; said Chairman Inez Tenenbaum. &quot;Drowning is still the leading cause of unintentional deaths with children younger than 5. That&apos;s why the Pool Safely campaign is encouraging all parents and caregivers of children, especially African American and Hispanic children, to help them learn to swim and to take water safety seriously.&quot;</p>

<p>An annual average of 390 pool or spa-related drownings for children younger than 15 occurred from 2007 to 2009; about 75 percent (293) of the reported fatalities involved children younger than five.</p>
<p>An estimated annual average of 5,200 pool or spa-related emergency department-treated submersion injuries for children younger than 15, from 2009 to 2011; children younger than 5 represented 79 percent, or 4,108, of these injuries.</p>
<p>Children between the ages of 1 and 3 (12 months through 47 months) represented 66 percent of estimated injuries for 2009 through 2011 and 67 percent of the reported fatalities for 2007 through 2009 involving children younger than 15 years.</p>
<p>The majority of the estimated emergency department-treated submersion injuries for 2009 through 2011 and the reported fatalities for 2007 through 2009 were associated with pools.</p>
<p>Approximately 51 percent of the estimated injuries for 2009 through 2011 and 73 percent of the fatalities for 2007 through 2009 involving children younger than 15 years old occurred at a residence.</p>
<p>Residential locations dominated incidents involving victims younger than 5 years of age (54 percent for injuries and 85 percent for fatalities).</p>
<p>Approximately 58 percent of fatalities (annual average of 226) occurred in in-ground pools. Portable pools accounted for 10 percent of the reported fatalities (annual average of 40) to children younger than 15 years of age.</p>
<p>There were no reported entrapment fatalities for 2011. CPSC received seven reports of entrapment injury incidents during 2011.</p>
<p>Today, Chairman Tenenbaum shared simple steps to pool safely and data from CPSC&apos;s annual drowning/near-drowning and entrapment reports, at an event at the International Swimming Hall of Fame in Ft. Lauderdale. She was joined by Wanda Butts, a mother who lost her son to drowning in 2006 and formed The Josh Project to help other children learn how to swim; Kim Burgess from the National Drowning Prevention Alliance and the Broward County Health Department&apos;s Drowning Prevention Coordinator; and USA Swimming&apos;s &quot;Make a Splash&quot; official Kim O&apos;Shea.</p>
<p>&quot;After losing my son, I wanted to do something to help others, so other moms wouldn&apos;t have to suffer from the loss of a child drowning,&quot; Butts said. &quot;Together, we can make a difference this summer and ensure that our kids pool safely.&quot;</p>
<p>&quot;The National Drowning Prevention Alliance has been a partner of the Pool Safely campaign since its inception.&quot; Burgess said. &quot;Education is the key to preventing tragic incidents at the pool this summer. We encourage everyone to Pool Safely this holiday weekend and in the months to come.&quot;</p>
<p>&quot;The USA Swimming Foundation is proud to lend its research and resources to further the life-saving learn-to-swim message,&quot; O&apos;Shea said. &quot;We are incredibly proud to have enrolled more than 1.2 million children in swimming lessons through our Make a Splash initiative, to educate parents and communities across the nation about the importance of learning to swim.&quot;</p>
